Abstract Cutaneous metastasis of lung cancer is a rare event and usually portends a grim prognosis. Here we describe a unique case of cutaneous metastatic lung adenocarcinoma largely limited to the epidermis, mimicking Paget's disease or a cutaneous adnexal tumor.
